Location: Fall Creek Health and Rehab. Arrived at approximately 0800.
Patient of focus: 65-year-old african american male, who admitted last friday for possible longterm care with primary diagnosis of pneumonia, respiratory failure and end stage renal disease. History of Diabetes Mellitus type 2, Tracheostomy, Peg-Tube, Dysphagia.
Patient alert but none-nonverbal. Identify verified by wristband.
My observations:
Upon entering room, Patient alert but not able to follow command. Tube feeding machine and tracheostomy supply at bedside.
Patient receives nothing by PO.
AV Fistula present in left arm for dialysis.
Upon performing a head to toe assessement, three wounds are present (left heel, right heel and sacrum.) treatment per chart is calcium alginate.
Per chart, he is homeless and no family present since hospital admission.
Vital Signs – Stable
Blood sugar noted 101 – Patient currently receives short and long-acting insulin.
