Introduction
This learning activity is intended to create an awareness of the various sources of human knowledge by providing you with an opportunity to question the research or evidence base of standard nursing interventions. The goals of this assignment are for students to: a) identify the bibliographical and/or other sources used in the development of a selected nursing technique/protocol, b) determine whether or not the nursing technique/protocol was based on research or other types of evidence, and c) suggest potential issues that may arise from using nursing techniques/protocols, which are not based on valid evidence.
The Learning Activity
Part I: Clinical Activity
Select a nursing protocol/technique (such as urinary catheterization or intravenous maintenance) from a clinical agency’s policy and procedure manual. (You may obtain a copy of the selected protocol from an agency’s nursing, nursing education or performance improvement office.) Then determine who wrote the protocol (procedure). This information may be obtained from an educator instructor, performance improvement specialist and/or nursing administrator. Request an interview with one or more individuals who are involved in developing practice protocols for the agency. The purpose of the interview is to determine the basis for the existing protocol. Suggested interview questions include the following:
Can you explain how nursing procedures/protocols are written in your agency? Who is responsible for writing them?
On what information was this specific protocol based (the one you chose to review)?
If references are not cited on the protocol, ask: “Are there any references that will give me a better understanding of the rationale for the practices included in the protocol?”
If references are listed on the protocol itself, ask: “What type of references are these?”
What kind of evidence did you use to develop the protocol?
After the interview, write a paper (no more than 4 double spaced pages) that: a) describes the technique/protocol and the process of its development, b) categorizes the sources of knowledge used to develop the procedure/protocol that were identified by the interviewee (i.e., authority, tradition, experience, or scientific), and, b) states a conclusion about whether or not the protocol is evidence-based and the specific evidence you found to support it – or not.
Part II: Library Activity
Select a nursing technique/protocol, which may be the same one as you chose for Part I, from a nursing textbook. Then answer the following questions:
Is there any mention made about the source from which the technique/protocol came?
Are references related to the technique/protocol cited in the text?
If the answer to either of the above questions is yes, list the references.
Categorize the references as follows: textbooks, nursing journals, non-nursing journals, unpublished source (e.g., doctoral dissertation), magazine article, and manufacturer’s instructions. You may add any category you think is not addressed above. Then determine whether each reference on the list is a primary or secondary source.
After addressing all of the above questions, write a summary (one or two paragraphs) of your findings, which you have documented throughout your paper. Then write a conclusion, which includes your evaluation of how much confidence you have in the validity of the protocol you chose. Be sure to give a rationale for your evaluation. Finally, describe your reaction to this learning activity.
