Beverly was a 45-year-old
woman who was an elementary school teacher for 20 years.  She loved
teaching, and she loved children. She was a single mother struggling
financially to support 14-and 17-year-old daughters. She was active in her
church and sang in the choir.  She was, in many ways, typical of many
adults with vocal nodules: she had a job that required excessive use of her
voice, sometimes with vocal abuses such as yelling to children on the
playground, and she had a variety of stresses in her life from financial
stresses from trying to raise two teenage children. Beverly tried to reach the
extremes of her vocal range during choir practice, choice performances and
singing around the house, and she had little time to take care of herself and
relax. She felt guilty when she tried to do something for herself even though
she felt chronically tense.
Besides symptomatic voice
therapy and vocal hygiene (conversation) education, the SLP tried to encourage
Beverly to learn how to take care of herself. She eventually began to
appreciate that if she “used herself up and burned herself out,” she would
not be able to continue teaching or singing, and she may have difficulty using
her voice when trying to communicate with her family and friends while teaching
and in all other environments. As she learned to balance her life a little
better, some of her generalized tension, as well as her vocal fold tension,
began to diminish.

would you say if the client asked you for specific advice, for instance,

work on my voice?”
