Evaluation section must include a plan for evaluating the effectiveness of the intervention, including,

A timetable for evaluating both the process and the outcome.
Conclusion
Concluding thoughts – overall effectiveness of the program. Expresses insights on working with the community and the role the nurse plays in the community.
